In accordance with the terms of this consent to the processing of personal data, the Service User by filling out and submitting an electronic form on the website https://www.petrukha.com/ confirms that he has read and agrees with the terms of the Public Offer and in accordance with Art. 11 of the Law of Ukraine “On the Protection of Personal Data” dated 01.06.2010 No. 2297-VI voluntarily provides the individual entrepreneur Petrukha Tatyana Vladimirovna and his authorized representatives and officials with her consent to the processing without any restrictions of the User’s personal data specified in electronic form and voluntarily provided by the User for the purpose of processing, in the personal data database "Clients".
Consent to the processing of personal data is provided by the User in order to ensure the implementation of civil law relations, as well as relations in the field of advertising and marketing, in accordance with the Civil Code of Ukraine, the Law of Ukraine “On Advertising”, the Law of Ukraine “On Protection of Consumer Rights”, as well as other regulatory legal acts.
The User grants the Contractor and/or any third parties the right to provide him with any commercial and/or informational information about the activities of the Contractor and/or any third parties by email, telephone and/or SMS, as well as any other means of communication.
The User also confirms his consent to the Contractor’s transfer of his personal data included in the “Clients” personal data database to personal data managers and any third parties, solely for the purpose stated above. The user does not require receiving a message about the transfer (distribution) of his personal data included in the specified personal data base to managers and third parties.
Consent to the processing of personal data is provided to the Contractor for an indefinite period.
By filling out and sending the electronic form to the Contractor, the User confirms that he has been notified of the inclusion of his personal data in the “Clients” personal data database owned by the Contractor, the purpose of collecting and processing personal data (according to the purpose specified in this document), as well as his rights provided for in Art. 8 of the Law of Ukraine “On the Protection of Personal Data”, according to which the subject of personal data has the right:
1) to know about the sources of collection, the location of his personal data, the purposes of their processing, the location or place of residence (stay) of the owner or manager of personal data or to give an appropriate instruction on the receipt of this information by persons authorized by him, except in cases established by law;
2) receive information about the conditions for providing access to personal data, including information about third parties to whom his personal data is transferred;
3) access to your personal data;
4) receive no later than thirty calendar days from the date of receipt of the request, except in cases provided for by law, a response on whether his personal data is being processed, as well as receive the content of such personal data;
5) present a reasoned demand to the owner of personal data with an objection to the processing of their personal data;
6) make a reasoned request to change or destroy your personal data by any owner and manager of personal data if this data is processed illegally or is unreliable;
7) to protect their personal data from illegal processing and accidental loss, destruction, damage due to deliberate concealment, failure to provide or untimely provision, as well as protection from the provision of information that is unreliable or discredits the honor, dignity and business reputation of an individual ;
8) file complaints about the processing of your personal data to the Commissioner or to the court;
9) apply legal remedies in case of violation of the legislation on the protection of personal data;
10) enter clauses restricting the right to process your personal data when providing consent;
11) withdraw consent to the processing of personal data;
12) know the mechanism for automatic processing of personal data;
13) to be protected from an automated decision that has legal consequences for him.
